I'm moving to a new location address. What do I do now?

In order to complete an address change, please contact our team with the following information.  

  1. What is the date of your move?
  2. What is your full new address?
  3. Will the new address be used as the mailing address also?
  4. What is the total square footage of the new address?
  5. If you will be purchasing equipment for your new location, do you want to increase this amount?  If so, what is the new total amount of coverage you would like?  As a reminder, this should be the actual cash value of the items at your facility including buildouts, flooring, equipment, technology, etc.  It cannot include the value of the building structure itself.
  6. With your move to the new location, do you want to adjust your Business Income and Extra Expense limit?  If so, what is the new total amount of coverage you would like? 
  7. Does your new landlord require a certificate of insurance?  If so, please provide the entity name and full address of your new landlord.
